- Faith Formation
  
  The Religious Education program works in partnership with parents and the parish community on the religious formation of children and families. Parents are the primary influence on the religious development of children, and the role of parents is honored in the Faith Formation process. Faith Formation classes takes place on Wednesday evenings from 6:00 pm - 7:30 pm. If you would like to register your child/ren for Faith Formation classes, please see form below.
Responsibilies

- Lead Catechist
  Prepare lesson plan in accordance with assistance from the teacher's manual provided.
  
  Liturgical Calendar and weekly Mass Readings should be included in the lesson plan.
  
  Have classroom or area prepared for class NLT 6:00 pm.
  
  Class will last 1 hour and 30 mins, from 6:00 pm - 7:30 pm.
  
  Ensure you retrieve all the student's books and appropriate supplies for class.
  
  Take attendance of the class and Catechists.
  
  Mark Attendance in the class's gray folder.
  
  Inform Faith Formation Coordinator if classroom supplies or if more instructional material is needed.
- Sacramental Catechist
  First Communion
  
  Follow all responsibilities listed under Lead Catechist in addition to the following.
  
  Assist the Faith Formation (FF) Coordinator with a Parent/Teachers meeting to discuss First Communion Retreat and First Communion Mass schedules and Baptismal Certificate requirement.
  
  Plan and hold a First Communion Retreat.
  
  Prepare the students for their First Reconciliation.
  
  Plan and hold practices.
  
  Ensure students understand and practice the examination of conscience.
  
  Ensure students memorized required prayers.
  
  Sign of the Cross
  
  Our Father
  
  Hail Mary
  
  Glory Be
  
  The Act of Contrition
  
  Ensure the students understand the meaning and purpose of the Blessed Eucharist.
  
  Practice taking Holy Communion with the students.
  
  Practice the precession and all parts of the Mass.
  
  Students will follow a dress code for their First Communion Mass.
  
  White dress/shoes and veil (optional) for girls and formal attire for boys.
  
  Confirmation
  
  Follow all responsibilities listed under Lead Catechist in addition to the following.
  
  Assist the Faith Formation (FF) Coordinator with a Parent/Teachers meeting to discuss event schedules and Baptismal Certificate that includes the notation of the Sacrament of First Communion.
  
  Ensure the student’s sponsor fulfills the requirements.
  
  Must be a confirmed Catholic.
  
  At least 16 years old
  
  Be practicing members of the Church.
  
  If married, they must have been married in a Catholic Church.
  
  Parents are not allowed to be sponsors for their own children, although godparents are permitted and encouraged.
  
  Regular interaction with student
  
  Help learn the following prayers:
  
  Sign of the cross
  
  Glory Be to the Father
  
  Hail Mary
  
  Our Father (Lord’s Prayer)
  
  Act of Contrition/Prayer of Sorrow
  
  Apostle’s Creed or Nicene Creed
  
  Ten Commandments

  Order of the Mass
  
  Gifts of the Holy Spirit
  
  Beatitude
  
  Candidates will complete the Confirmation Name booklet on their chosen Confirmation Saint.
  
  Candidates will complete the Letter to the Bishops essay describing what confirmation means to them and why they want to be confirmed.
- Assistant Catechists
  Follow all responsibilities listed under Lead Catechist in addition to the following.
  
  Assist Catechist with monitoring the classroom and correct behavior.
  
  Assist with class projects and crafts.
  
  Assist with set-up before class and clean-up after class.
  
  Assist students if they require additional help.
  
  Act as lead catechist in the absences of the lead catechist.
- Substitute Catechist
  When notified by FF Coordinator, they will report to Sacred Heart Office and receive appropriate grade’s teacher manual.
  
  Prepare lesson plan in accordance with assistance from the teacher’s manual provided to the best of their ability.
  
  Liturgical Calendar and weekly Mass Readings should be included in the lesson plan.
- OCIA Assistant
  - Teach course in accordance with the Diocese of Green Bay OCIA Manual.
  Ensure candidates fill out and turn-in the Diocese of Green Bay OCIA Information Forms are require.
  Ensure the candidate’s sponsor fulfills the requirements.
  Must be a confirmed Catholic
  At least 16 years old
  Be practicing members of the Church
  If married, they must have been married in a Catholic Church.
  Parents are not allowed to be sponsors for their own children, although godparents are permitted and encouraged.
